What to Expect For My First Appointment?
Becoming a Family Medicine Patient:
You don’t need a referral to become a family medicine patient. New Patients can expect an appointment within one day and in some cases, same day.
On your initial visit, please expect to:
Fill out New Patient Forms (also found in the patient portal and may be filled out ahead of time), which also includes, Health History and Acknowledgement of Privacy Practice Forms
Insurance related Forms. Your up to date Insurance Card MUST BE PRESENT together with a VALID form of Identification such as your driver’s license.
Pay your co-pay. You are responsible to know how much your co-pay is for an office visit to see your primary care physician. Our office is obliged to collect co-pay at the time of your visit. Please help us comply with Insurance regulations by paying your co-pay at the time of visit.
Sign Forms to release Medical records from previous doctor’s office as necessary
